Westport Man Accused of Forging Prescription at CVS

Former Fairfield resident Bradley Jack -- owner of the most expensive residential property in Fairfield -- was charged with second-degree forgery.

Bradley H. Jack, 53, of 290 North Avenue, Westport, was charged Friday with second-degree forgery after he turned himself into police Westport Police Department on a warrant stemming from an incident in November 2011.

At that time, Jack allegedly forged the date of a prescription for a controlled substance at which was made out to him by a doctor from Fairfield, police said. He posted $5,000 bond and was released for court March 21.

Jack was accused of a similar crime in June, 2011 where he allegedly tried to obtain Oxycontin and Ritalin from CVS.

Bradley is reportedly owner of the most expensive residential property in Fairfield  -- a 20-acre waterfront estate on Sasco Hill Road in Fairfield, which he owns with his ex-wife, Karin Jack. The two are reportedly separated and Jack currently lives in Westport.
